Exercise 7 – Internal Safety Investigation

Divide into three groups and use a different organisation.
Consider the following incidents. It has been decided each require investigation.
Use imagination and group discussion to create a safety trend and hence the need for an investigation.
Use the example from class for guidance if required.
Situation one – BabyQ Airlines
Four times this year there have been significant airspace infringements into Melbourne airspace (Significant enough that Airservices have reported and CASA have called to request an explanation). The AIs have all occurred late at night following long days conducting multiple RPT operations to and from King Island.
Situation two – SlingWing Scenic
There continues to be FOD issues and subsequent damage to aircraft on taxi from the hardstand prior to takeoff. It is always occurring when the opposition is also conducting operations, and the workload tempo is high. While relations with the other business are generally good, there is always an amount of ‘jostling’ for position prior to takeoff as the pilots negotiate separation.
Situation three – Mick and Millie Maintenance
There has been a consistent stream of aircraft owners returning their aircraft to the hanger complaining of minor problems that could have caused significantly more. (The latest issue was an oil cap had not been fastened.) This has been occurring since the new roster system changed to include an early and a late shift. Previously there had only been one day shift.
Situation four – Seeby flying training
Seeby are experiencing multiple instances were students are getting lost on Navigation exercises. The problems are exacerbated by language barriers between the students and ATC as well as other aircraft trying to assist. Student maps, the preparation for the flights as well as the living and working arrangements are examined as part of the investigation.
Situation five – Paradise airfield
There has been multiple instances of runway incursions by work vehicles during RPT operations to Paradise airfields. In several instances the RPT operator has submitted Aviation Incident reports to ATSB which has contacted the airfield management to investigate. The incursions are most common by the primary earthworks contractor. All vehicles are required to carry aviation VHF radios and both the FIR frequency and CTAF are in use at the airfield.
